He comes up with the vocal line, sings gibberish over it and then the lyricist (Geezer Butler in Sabbath, Bob Daisley in the 80's, etc etc in the 90's, Kevin Churko these days) writes the words, with Ozzy perhaps chipping in a line and or coming up with the title. Guessing it would have been around the turn of the millenium.
