The first movie is called First Blood, no problem there. The second movie became Rambo: First Blood part II. Check out this video at around 3.50 http://cinemassacre....-sequel-titles/First Blood is First Blood, Rambo: First Blood part II is thusly named because it's a continuation of the Nam theme of the first one 'do we get to win this time?', Rambo III departs from that cuz it's no longer rolling with the Nam thing (it's in Afghanistan, isn't it?) and the 4th one is called Rambo or John Rambo, not just Rambo and the reason for that is the personalisation, JOHN Rambo, as in him the person instead of him the killing machine, with it coming full circle at the end and him coming to terms with his identity and not just his killing machine thing, as illustrated by his moseying on back home at the end of the film.
